4 You have a strong head-chest-hip connection, and move through the water smoothly because of that.
Breathing

3 Either starting to breathe a little late in the stroke cycle or taking too long to breathe. The result is that you are pressing down on the water with that leading arm and losing some stroke length.

4 Very good rotation to both sides.
Arms

3 Extension: Very good, but takes a bit too long to get there because of hand entry a bit shallow and in front of head.
Catch: Deep and a little early, which is more like a sprinter than an endurance swimmer
Pull: Good early pull but the end is lost since the arm is under the body and slips to the side as the pull completes
Recovery: You recovery your arms very low to the surface and in an arc, causing your hand to cross in front of your head.

Arms. Try to enter your hands at 10 o'clock and 2 o'clock, enter them a little more steeply, and drive them to full extension a little sooner. Then glide just a moment longer than you are doing, and try to pull more to the side than under your body.

Breathing. Try to roll your face to the air sooner, or breathe a little more quickly. Entering your arms as described above may make this easier.

Kicking. You kick very well, but be aware of when your legs are getting deep in the water - and that might help you to identify any "dead spots" in your stroke.
